EOS Family Dental takes your privacy seriously. This Privacy Policy explains how we handle personal information collected through our pre-opening website and EOS Opening List.

At this stage, the EOS Opening List collects only your email address when you choose to provide it. We do not ask for health information, treatment information, insurance information or other patient records through this form.

We use your email address to manage the EOS Opening List and to let you know when EOS Family Dental opens and appointments become available. We will not use your email address for unrelated purposes without your consent.
Consent and unsubscribing
Joining the EOS Opening List is voluntary. By submitting your email address through the signup form, you consent to receiving the opening and appointment-availability communications described on the page. You may withdraw your consent and unsubscribe at any time using the unsubscribe option included in our emails.

Patient and health information
The EOS Opening List is not a patient-registration or clinical communication system. Please do not use the Opening List to provide personal health information. Information collected in connection with future dental care will be handled through the appropriate patient and clinical systems and will be subject to the privacy requirements applicable to dental practices in Ontario.

We may update this Privacy Policy as EOS Family Dental's website and services develop. The current version will always be made available on this website.
